Hi,

I'm running ES and Kibana 6.8.0, and using the 7.0 upgrade assistant, I'm trying to reindex some old Logstash indices created in ES 5.x. It fails due to the presence of the `include_in_all` field in the index mappings:

```
[mapper_parsing_exception] [include_in_all] is not allowed for indices created on or after version 6.0.0 as [_all] is deprecated. As a replacement, you can use an [copy_to] on mapping fields to create your own catch all field
```

The indices in question haven't been written in ages, and won't be either. The `include_in_all` is `false` in all the cases I've seen. How do I get rid of it altogether?
