ElasticSearch NEST combining AND with OR queries

I'm not sure if this is an appropriate place to post a question on using C# NEST (5.2) to generate an ElasticSearch Query. I'm having a ridicious time generating Nest to output a specific ElasticSearch query.

I do have an elastic search query that works - so I'm not sure if a curl is appropriate.
