DB Hostname is missing (unnamed)

It is very important to show the host name during a database transaction.

Good timing! We've just started discussing adding the destination address to span context: https://github.com/elastic/apm/issues/115.

The idea is that agents would record the address of the database server in span context, and then you would be able to see that by clicking on the database span, bringing up the span details.
